Position Overview:
The primary responsibility of the Memory Care Concierge it to serve as the principle point of contact for residents, families, and guests, ensuring a warm, welcoming, and engaging environment. This role combines hospitality, social services support, and activity coordination to enhance the quality of life for residents living with dementia and other memory-related conditions. The individual will also fulfill Social Services Designee (SSD) responsibilities, advocating for residents’ psychosocial needs, and oversee activities programming to provide meaningful engagement and stimulation.
Essential Job Functions:
Concierge & Resident Experience
- Greet and assist residents, families, and visitors with exceptional hospitality and professionalism.
- Foster a warm, home-like atmosphere, ensuring residents feel supported and valued.
- Serve as a liaison between families, residents, and staff, addressing concerns and coordinating communication.
- Assist with move-ins and transitions, providing orientation and emotional support to residents and families.
Social Services Designee (SSD) Responsibilities
- Practice Comfort Matters philosophy throughout the campus, with all resident interactions and staff development.
- Advocate for residents' rights, dignity, and well-being in collaboration with nursing and administrative teams.
- Conduct psychosocial assessments and care plan meetings to ensure residents' needs are met.
- Provide emotional support and resources to residents and families, assisting with challenges related to dementia and aging.
- Coordinate with healthcare providers, therapists, and social workers to enhance resident care.
- Maintain accurate documentation and comply with state and federal regulations related to social services.
- Assist residents’ campus-wide with various social services duties as they arise.
Activities Coordination, Level 5 Assisted Living
- In collaboration with Life Enrichment Manager and team, organize, and lead engaging activities tailored to memory care residents, focusing on cognitive stimulation, socialization, and physical well-being.
- Develop and implement individualized activity plans that align with residents' interests and abilities.
- Facilitate group and one-on-one activities, such as music therapy, art, sensory programs, and reminiscence sessions.
- Collaborate with caregivers and volunteers to ensure meaningful resident engagement.
- Communicate upcoming events to residents, families, and staff.
Qualifications and Experience:
- Must be at least eighteen [18] years of age.
- Must complete CNA training within 6 months of hire date.
- Must have strong interpersonal and communication skills, with a passion for working with older adults.
- Must have high school diploma or GED, preferred experience in Social Work, Gerontology, or a related field.
- Prefer certification or training as a Social Services Designee (SSD).
- Prefer experience in memory care, senior living, or a related healthcare setting.
- Prefer knowledge of dementia care approaches and best practices.
- Prefer prior experience leading group activities or one-on-one engagement.
Physical Requirements:
Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Requires walking or standing to a significant degree; or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
