Kidron Bethal Village is looking for a full-time Social Services Designee/Life Enrichment Coordinator to join our team!
Position Overview:
The primary responsibility of the Social Service Designee/Life Enrichment Coordinator (SSD/LEC) is to assist the Social Worker with planning, implementing and evaluating social service programs (60%). The Social Service Designee/Life Enrichment Coordinator will also assist Life Enrichment staff with facilitating activities in Assisted Living (40%). The SSD/LEC assures that on-going programs are in accordance with federal, state, and local standards, company policies and procedures, and designed to support the needs, interests (physical, mental, and psychosocial) of the residents.
Essential Job Functions:
Building Relations
- Interview residents/families as necessary in a private setting.
- Involve residents and family in planning programs when possible.
- Assist resident in adjusting to social and emotional aspects of stay in the facility.
- Refer residents/families to the appropriate social service agencies when the facility does not provide the services or needs of the resident.
- Communicate with residents/families regarding upcoming care plan meetings.
- Assist with mealtime activities, including escorting residents to the dining room and passing trays.
- Facilitate monthly Resident’s Council meetings and as requested by the residents.
Administrative Procedures
- Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Administrator as required.
- Complete a social history assessment on all residents within 7 days of admission.
- Participate in the Minimum Data Set (MDS) process and complete quarterly social service assessments.
- Coordinate the provision of social services to residents/families and significant others, enable them to understand the impact of illness on individual family functioning and to achieve maximum benefits from health care services.
- Assist in effectively maintaining all required records for the Social Services Department; demonstrates good organization skills.
- Participate in quality improvement activities of the Social Services Department to promote the maintenance of high-quality resident care.
- Demonstrate and promote an awareness of resident’s rights; function as a resident’s advocate; ensure staff members are aware of resident’s rights.
Compliance Matters
- Work with consultants to implement recommended changes. Coordinate activities with other departments as necessary.
- Assist in coordination of the volunteer program, resident shopping and outings. Assist with supervision of volunteers.
- Complete required paper work for each resident and maintain records as required by State and Federal regulations.
- Work with Senior Living Advisor Team (SLAT) and AL Nurse Manager to coordinate admissions (including assessments and paperwork) and “orientation” for all new residents.
- Keep current with federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Life Enrichment Director/Manager.
- Provide Social Services to residents and maintain social service paper work as required by State and Federal regulations.
Coordinating Activities
- Assists AL nurse manager with regularly scheduled resident assessments and reviews.
- Participate in surveys (inspections) made by authorized government agencies. Assist in reviewing and developing a plan of correction for activity deficiencies noted during survey inspections.
- Arrange transportation for field trips when necessary and escort residents on field trips.
- Assist in evacuation of residents during drills or actual emergencies.
- Schedule medical, dental, and vision appointments for residents as necessary, including transportation and necessary nursing assistance.
Qualifications and Experience:
- Must be at least eighteen [18] years of age
- Prefer Social Service Designee certificate or be willing to obtain within 6-months of hire date
- Must be willing to obtain Certified Nurse Aide certificate within 12-months of hire date
- Prefer a minimum of one-year of experience in a social services department, in a skilled nursing care facility, or other related medical facility
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset and hostile residents
- Must be able to hold professional conversations with residents and family members regarding transitions, counseling and emotional support
Physical Requirements:
Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Requires walking or standing to a significant degree; or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
