This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Alachua County. The responsibilities include case management for Healthy Start clients and education and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines. Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.

ESSENTIAL FUNCTIONS:

  • Perform case management services for individuals/families receiving Healthy Start services.
  • Assist clients in accessing community services and provides information and education to clients about smoking cessation, childbirth education, breastfeeding support, parenting and infant growth and development.
  • Initiate tracking procedures for client, family of client and client’s provider to manage appointments and referrals for services.
  • Maintain/update client files, tickler cards and ensure Healthy Start database is updated.
  • Assess the educational needs of Healthy Start clients who are at risk for poor pregnancy outcomes.
  • Assist with providing office coverage and be available to receive any phone calls or assist any visitors to the Healthy Start Office during required office hours.
  • Complete all Healthy Start assessments and home visits as required per program guidelines.
  • Other duties as assigned.

QUALIFICATIONS:

  • Education/Experience

Bachelor’s degree in Social Sciences, Social Work, Human Services or a Health Related Field such as Health Education, Child Maternal Health or Public Health is required. 2 years of related experience preferred. Experience conducting home visits with similar population preferred. Bilingual (Spanish) preferred.

  • Licensure/Certifications

Valid Florida drivers license and insurable by agency’s insurance carrier.

  • Other Requirements

Must successfully clear Level II Background Screen

Must possess an acceptable driving record and have reliable transportation.

KNOWLEDGE, SKILLS & ABILITIES

  • Excellent knowledge of community resources
  • Excellent interpersonal and communications skills (verbal and written)
  • Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
  • Effective public speaking skills
  • Ability to travel up to 75%
  • 100/300 insurance is required if transporting clients in personal vehicle
  • Ability to lift 10-25 pounds
  • Ability to safely operate a motor vehicle
  • Ability to work evenings and weekends as needed
  • Ability to work in an environment with frequent interruptions and a moderate noise level
  • Ability to independently complete job duties within required time frames
  • Knowledge of family and systems theory
  • Knowledge of obstetrics, pediatric health, and health care systems preferred

The employee must also frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. The employee is regularly required to bend/stoop and kneel. Repetitive motion is also required. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
