The Family Preservation Case Manager is responsible for the provision of case management and support services for families in Circuit 5 and works in a team approach with the Family Preservation Specialist. This includes the completion of referrals to assist and stabilize the family which includes but are not limited to mental health services, psychiatric services, housing, or other community referrals. Coordination with other agencies and community services is also required. Services are provided in the home to meet the unique needs of the families served.
ESSENTIAL FUNCTIONS FOR FAMILY PRESERVATION CASE MANAGER :
- Provides direct services to children and their families which may include :
- Reviews comprehensive assessments with the family
- Reviews treatment plans and contracts for services with the family
- Establishes goal setting with families
- Manages and ameliorate crises with families
- Participates in discharge planning, which may include plans for follow up treatment and connection to community services and programs
- Provides case management services
QUALIFICATIONS :
Education / Experience
Bachelor's Degree in the human services field or BSW required and three years' experience in a comparable position in a human services agency preferred but not required.
Licensure / Certifications
Current Background Clearance Screening Letter
Valid Florida driver's license
Proof of vehicle insurance
