JOB SUMMARY:

This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Lake County. The responsibilities include case management for Healthy Start clients and education and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines. Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.

ESSENTIAL FUNCTIONS:

  • Answer telephones, schedule appointments, and take and relay messages in a professional and timely manner.
  • Complete purchase requisitions as needed.
  • Provide Office coverage and be available to receive any phone calls or assist any visitors to the Healthy Start Office during required office hours.
  • Maintain the organization of supplies and with approval replenishes as necessary.
  • Complete initial contacts as required by the Healthy Start Standards and Guidelines and core performance measures.
  • Under the supervision of the Healthy Start Director, they may provide direct support to clients by providing resource and referral.
  • Under the supervision of the Healthy Start Director may conduct home visits for the purpose of providing assistance and report any problems or concerns to director and assigned care coordinator.
  • Other duties as assigned.

QUALIFICATIONS:

Education/Experience

  • Associate’s degree required, preferably in human service-related field
  • Two years’ experience working with families preferred.
  • Bi-lingual (Spanish) required

Other Requirements:

  • Valid Florida driver’s license and insurable by agency’s current insurance carrier
  • Must carry personal phone with voice plan only for business communications during and outside business hours

KNOWLEDGE, SKILLS & ABILITIES:

  • Excellent knowledge of community resources
  • Excellent interpersonal and communications skills (verbal and written)
  • Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
  • Effective public speaking skills
  • Ability to travel up to 45%
  • Ability to safely operate a motor vehicle
  • Ability to work evenings and weekends as needed

The employee frequently is required to stand; walk; use hands to finger, handle or feel; and to reach with hands and arms. The employee must also frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 20 pounds.

The employee is regularly required to bend/stoop and kneel. Repetitive motion is also required. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
