Job Summary
The Case Manager, Residential is responsible for the coordination and completion of all tasks, documentation and communication relevant to the provision of case management support service for clients beginning with pre-admission and ending with discharge.
Job Duties
- Upon admission, create client master chart and summary sheet; meet with family and caseworker as necessary and provide applicable information including handbook; review admission demographics and other pertinent information with family and casework to insure accuracy; retrieve any documents not presented to admissions department; take required photograph of client
- Notify all current providers of admission within twenty-four (24) business hours of admission; respond to all customer requests within forty-eight (48) business hours
- Schedule all necessary meetings including all case conferences, "red flag" meetings, ITT, reauthorization, CASSP and Discharge Planning and ensure that all necessary participants are invited. Attend when required and assure all attendees at meetings are properly documented, as well as those not in attendance. Document and distribute completed treatment plans to all invitees and attendees. Notify Aftercare Specialist of changes in discharge plan and / or projected date of discharge
- Track last date of authorization by MCO and date reauthorization packet needs to be sent to MCO to insure uninterrupted authorizations. Insure reauthorization packet is completed and returned to MCO within specified time frames
- Complete "level one" audits on at least two charts weekly and submit results to all applicable personnel and leadership. Complete chart audits on all designated clients prior to being reviewed by outside auditors
- Ensure that Release of Information data and forms are accurate and up-to-date
- Track annual psychiatric evaluations and biopsychosocials and report the need to complete to applicable personnel
- On date of discharge, meet with client and parent / guardian and ensure that all applicable documents and information are shared and signed
