Position Overview: KidsPeace New England is seeking a dedicated and compassionate Case Manager to join our team. In this role, you will identify the medical, social, educational, and other needs of eligible families, facilitate access to necessary services, and manage caseloads of children and families.
Key Responsibilities:
- Conduct intake, assessments, and develop comprehensive care plans.
- Schedule regular meetings with assigned families to provide support, resources, and guidance.
- Coordinate services with clinical, educational, and healthcare providers.
- Maintain accurate documentation in compliance with quality assurance standards.
- Communicate effectively with treatment teams and other contacts regarding client needs.
Qualifications:
- Bachelor’s Degree required
- Relevant human services experience preferred
- Experience working with children and families preferred
