As a Family Consultant, you will play a key role in shaping the future of children in our community by identifying their needs and ensuring they have access to the services and support necessary to thrive.
Position Overview:
The Family Consultant is responsible for identifying the medical, social, educational, and other needs of eligible foster care families. You will take on a leadership role in foster care, guiding the planning and implementation of direct treatment and overall case management for children and families within our service area. This is a highly collaborative role, working alongside a dedicated team to ensure the best outcomes for the families you support.
Key Responsibilities:
- Assess the needs of foster care families and develop personalized care plans
- Connect families with essential services and resources
- Act as the team leader for assigned cases, overseeing treatment planning and implementation
- Ensure proper case management and coordination of services
- Provide ongoing support and advocacy for families throughout their foster care journey
Qualifications:
- Education & Experience:
- Bachelor's degree
- 2 years of experience working with children and families
