Job Summary:
The Mobile Outreach Response Engagement Stabilization (MORES) Team Licensed Clinician acts as the MORES Team Leader, providing therapeutic interventions and coordinating treatment services for each family in care. This role involves addressing the behavioral health care needs of children based on established treatment goals and Early and Periodic Screening, Diagnostic and Treatment (EPSDT) standards and state regulations.
Key Responsibilities:
- Assist the Team Leader in consulting with identified providers, engaging community and natural supports, and including their input in the MORES planning process.
- Participate in person-centered MORES planning (MP) process.
- Implement home-based crisis evaluation, stabilization, and support plans with youth and caregivers as indicated in the MP.
- Provide psychoeducation as indicated in the MP.
- Monitor and evaluate the effectiveness of interventions, evidenced by symptom reduction and progress toward goals identified in the MP.
- Provide crisis interventions as necessary.
- Ensure accurate and timely documentation meeting state requirements, including MCP, service notes, assessments, service authorization requests, and discharge planning/summary.
Requirements:
- Licensed Behavioral Practitioner in one of the following fields: Psychoanalyst, Clinical Social Worker, Marriage and Family Therapist, Mental Health Counselor, Master Social Worker
