Job description
Wyandot BHN is seeking a dedicated Case Manager to join our adult supportive housing team! The Case Manager provides goal directed intensive services to adults with severe and persistent mental illness (SPMI) for the purpose of increasing functional skills, decreasing the need for hospitalization, and maximizing independent functioning in the community. Services will be provided in the consumer’s home and in the community. In order to assist the consumer in maximizing independent living
Essential Job Duties:
- Conduct active outreach of consumers within the community.
- Establish a trusting relationship with the consumer
- Convey hope and respect
- Use motivational interviewing techniques to actively engage the consumer in services
- Identifies strengths and needs of consumers and works to involve them in recovery oriented planning for desired services.
- Works to fully integrate consumers into the community
- Identify and address barriers to recovery
- Works closely with families of consumers to provide education and support.
- Link the consumer to other agency resources (peer support, community integrations, vocational services, outpatient services).
- Link the consumer with community resources (educati
- onal, cultural, recreational and social activities).
- Works closely with SRS, SSA, and other agencies as appropriate to secure financial support.
- Assists consumers in accessing desired housing and promotes stability through on-going support.
- Provides crisis intervention as needed
Job Requirements:
- Possesses bachelor’s degree in social service field or related professional experience.
- Ability to recognize and be sensitive to cultural and ethnic differences.
- Ability to communicate directly and effectively in a cooperative manner.
- Ability to relate positively with colleagues and representatives of other agencies.
- Familiar with mental health theory and practice.
- Must possess appropriate written communication skills.
- Must be able to safely perform the essential job functions, with or without reasonable accommodations.
