Job Summary
Kind Hearts Hospice is opening a new branch in Mansfield in early 2025. We are a "boutique" Hospice, striving to do things differently from the corporate hospice experience.
Our founders worked for "the big guys" for years and grew tired of corporate policy coming before patient care and advocacy. Therefore we launched a new approach: small census, small staff, small coverage area. This model has worked extremely well for us over the last five years in our main branch in Toledo, Ohio.
Now we are opening our first satellite branch in Mansfield in the first quarter of 2025.
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team. In this role, you will be responsible for coordinating patient care, ensuring that individuals receive the necessary medical services, and facilitating communication between patients, families, and healthcare providers.
The ideal candidate will possess a strong background in hospice care, with experience both in facilities and homes providing end of life care. Your expertise will be vital in managing cases and providing high-quality care to patients with diverse medical needs.
Responsibilities
- Assess patient needs through diagnostic evaluation and vital signs monitoring.
- Develop and implement individualized care plans based on patient assessments.
- Coordinate with multidisciplinary teams to ensure comprehensive patient care.
- Provide education and support to patients and their families regarding treatment options and health management.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Maintain accurate documentation of patient interactions and care plans in compliance with hospital policies.
- Advocate for patients’ needs within the healthcare system, particularly in areas such as infectious disease care and critical care settings.
Requirements
- Current Registered Nurse (RN) license in Ohio.
- Previous Hospice experience of at least one year field work.
- Strong skills in diagnostic evaluation and the ability to interpret vital signs effectively.
- Excellent communication skills for effective collaboration with healthcare teams and patient education.
- Ability to work independently while managing multiple cases effectively.
- Hospice certification is a plus, but not required.
- Commitment to providing compassionate patient-centered care.
