Job Title : Registered Nurse (RN)

Job Description :

• Coordinate management of care to ensure optimal utilization of resources, service delivery, and compliance with external review agencies.
• Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
• Enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness.
• Integrate functions of case management, utilization review and management, and discharge planning.
• Act as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs.
