The Physical Therapist provides prescribed treatments of physical therapy, which involve a complex and sophisticated level of modalities designed to restore the patient to his/her maximum rehabilitative potential.
Identifies the problem and performs physical therapeutic treatments as prescribed by the patient's physician.
Assesses, reports, and records patient's status and progress on appropriate Agency laptop and to the health care team.
Ensures initial and on-going assessments to determine level of functioning and evaluation of patient/family response to therapy, including OASIS assessments at appropriate time points.
Plans, evaluates, implements and teaches patients and family members to provide for a continuous physical therapy program during care and discharged.
Participates in clinical conferences and in-service programs to share information and stimulate the health care team in meeting rehabilitation goals.
Delivers quality treatment services to patients in accordance with accepted standards of practice and effective for the condition, which the patient suffers.
Develops and revises a plan of treatment and care as indicated in collaboration and consultationwith the physician, ITC members, and other healthcare team to achieve goals that is not a duplication of active physical therapy programs.
Evaluates the effectiveness and outcomes of care and identifies barriers to desired outcomes.
Set goals related to the needs of the patient.
Maintains specialized hospice training, if necessary.
Schedules visits with patients according to assigned caseload in geographic location.
Prepares and maintains appropriate clinical and administrative records in a prompt and comprehensive manner, documenting patient limitations/interventions and progress.
Timely records all assessment and evaluation data, treatments and patient's response to therapy intervention. C
Completes paperwork within 24 hours of visit, and communicates daily.
Collaborates with Clinical Director to meet minimal productivity of 85%.
Develops and instructs Agency staff and family members in maintenance therapy programs and therapeutic treatments as approved by the physician.
Supervises and directs Licensed Physical Therapy Assistants and home health aides as appropriate in carrying out the plan of treatment (including LCD) as established by the therapist.
Assists with the orientation of new staff as needed.
Evaluates equipment needs to increase patient's functional and independence levels and assumes responsibility for equipment signed out for patient use.
Will be expected to travel to other offices to meet the demand of patient needs as instructed by Clinical Director.
Ensures referrals are evaluated timely with NPU goal of 6% or less.
Communicates daily with the Clinical Director or Team Leader (RN).
Attends appropriate number of inservices as required.
Adheres to the Agency's personnel policies and Compliance Plan.
Willingly performs other duties as assigned by supervisor.
Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
JOB REQUIREMENTS
Graduate of an approved Physical Therapy curriculum.
One-year of experience as a Physical Therapist required.
Physical Therapist currently licensed to practice physical therapy in North Carolina.
Maintain current CPR certification (BCLS)
