Overview
We are seeking a dedicated and compassionate Nurse to join our healthcare team. Duties
- Provide comprehensive nursing care to patients, including assessments, planning, implementation, and evaluation of care plans.
- Administer medications and treatments as prescribed, ensuring adherence to established standards of care.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
- Document patient information accurately using electronic health records systems such as Epic.
- Conduct MDS coding and maintain compliance with relevant healthcare regulations and standards.
- Educate patients and their families on health management, disease prevention, and wellness strategies.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Provide emotional support to patients and families during challenging times, particularly in hospice settings.
Requirements
- Valid nursing license in the state of Connecticut (CT).
- Experience working with children and individuals with developmental disabilities is preferred.
- Background in home care or hospice palliative medicine is highly desirable.
- Familiarity with MDS coding and healthcare standards is a plus.
- Proficient in using electronic health record systems, particularly Epic.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently as well as part of a collaborative team environment.

Pay: $25.00 - $31.37 per hour
Expected hours: 4 – 40 per week
