JOB SUMMARY
-
Assist patient and their families with issues that accompany illness or inhibit recovery and rehabilitation. Through the development of an individualize psychosocial and discharge planning assessment, the Medical Social Works collects and analyzes patient information to assist in understanding social, emotional and environmental factors underlying a patient’s illness. Will have a direct impact on discharge planning and patient throughput management.

DUTIES AND ESSENTIAL FUNCTIONS

Overall performance rating of below indicated essential job duties:
-
Complete individualized psychosocial and discharge planning assessments of patients to identify emotional, social, and environmental factors related to diagnosis, illness, treatment and/or life situation that impede or promote healthy transitions of care.

*Provide interventions as indicated in the completion of psychosocial and discharge planning assessments including but not limited to crisis intervention, education, advocacy, referral management and/or mediation.

*Assist with screening, identification and intervention in cases involving suspected abuse, neglect, intimate partner violence, exploitation and sexual assault.

*Collaborate with and provide education to the interdisciplinary treatment team regarding patients assessed needs, care transition options and patient choice; providing representation of the social work perspective in care plan development.

*Monitors, evaluates and documents patient progress according to measureable goals described in the individualized patient care plan ensuring plans remain updated based on the patient’s current needs.

EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS

Minimum requirement:
-
Master’s degree (MSW)
-
Kentucky State Licensure as Certified Social Worker (CSW)

Preferred qualifications:
-
2 years of related experience or training in health care setting

Kentucky State Licensure as Licensed Clinical Social Worker (LCSW)
