What are the responsibilities and job description for the Treatment Coordinator/Case Manager/Social Worker in Christian Environment position at Kingdom-Pathways?
A Treatment Coordinator/Case Manager/Social Worker is needed in a Christian homeless outreach in St. Paul, MN. 2,000 hours of experience with individuals with substance use disorders is required. A bachelors degree in one of the behavior sciences or related fields is required.
Candidates with experience as Social workers or case managers, or even experience as technicians or other positions with individuals having substance use disorders (2,000 total hours) are encouraged to apply.
Job duties include working with homeless clients on:
(1) Coordination of services between mental health service providers and Kingdom Pathways MICD treatment counselors to enable more effective therapy for both Kingdom Pathways MICD counselors and outside mental health service providers such as psychiatrists, psychologists, and other mental health workers.
(2) Coordination of servies between medical providers and Kingdom Pathways MICD treatment counselors, RNs, and Housing Staff to facilitation better health services provision for our homeless clients.
(3) Coordination with providers of Medication Assisted Therapy to ensure accurate dosage plans and better and more effective overall therapy to our homeless clients.
(4) Coordination with providers of insurance and other economic assistance, to assist our clients in maintaining their insurance and obtaining other economic assistance.
(5) Coordination with providers of various social services to assist our clients in obtaining all available social services assistance from the many providers of social services for the homeless.
(6) Coordination with providers of life skills assistance and educational assistance to assist our homeless clients benefit from anger management classes, GED, job training classes, and other educational resources available to our homeless clients.
(7) Coordination of the involvement of significant others in the treatment planning process whenever possible.
Service provision will include at least 1 hour a month of supervision by a Licensed Alcohol and Drug Counselor (LADC) and all service provision must be documented and included in the client file.
