What are the responsibilities and job description for the Vocational Rehabilitation Counselor position at KINGS COUNTY HOSPITAL CENTER?
SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conducts initial screenings and/or assessments of patients
- Facilities and/or co-facilitates individual, group, and family counseling; provides psychoeducation, conducts skills training.
- Assesses and evaluates the client's rehabilitation; vocational and work-readiness, educational and training needs.
- Implements and coordinates the treatment goals and objectives for patients with vocational and rehabilitation needs.
- Evaluates testing materials and administers vocational testing as appropriate
- Consults with multidisciplinary team and participates in required professional development and involvement in state and local services regarding procedures affecting rehabilitation services.
- Develops and continually monitors and updates, as indicated, treatment/recovery plans in collaboration with patients and members of the clinical treatment team, including establishment of short and long-term treatment goals, objectives, and interventions.
- Participates in the evaluation of treatment progress.
- Coordinates and provides case and care management services and activities; makes referrals and linkages.
- Collaborates in discharge and vocational transition planning. and collaborates with patient and clinical treatment team. May make field visits and/or accompany patients to community services.
- Maintains timely, accurate, and appropriate documentation in compliance with all applicable standards to ensure appropriate reimbursement. Accurately and timely records treatment planning, consistent with established standards, applicable administrative guidelines and regulatory requirements
- Provides orientation to new patients, patients' families and significant others; discusses financial responsibility with patients and provides money management assistance.
- Provides crisis intervention and support by responding to and following through in crisis situations; demonstrates good clinical judgement by seeking additional assistance and/or consultation in the management of crisis situations, when needed.
- Conducts outreach activities, provides advocacy, and acts a liaison between programs, providers and community supports.
- Participates in relevant staff meetings, case conferences and trainings; participates in special projects and internal/external quality assurance/performance improvement activities.
- Participates in community outreach activities on and off campus.
- Holds and maintains required certification(s) and completes required continuing education; submits appropriate documentation within established timeframe. Complies with NYC Health Hospitals policies and procedures, and applicable to local, state and national requirements and guidelines.
- Performs other related duties, as assigned.
- Demonstrates an awareness of the organization's safety plan
- Having a substance use background or CASAC certification is a plus
