JOB SUMMARY

Will Assess individuals referred for participation within Kore Grant and through Wellness Recovery consults. Assist individuals with issues that accompany illness or inhibit recovery and rehabilitation related to substance abuse. The LSCW acts as a liaison with community and private resources to reduce gaps in participants' healthcare. Will Collect and submit patient information in line with the Grant requirements.

Overall performance rating of below indicated essential job duties:
Completes individualized psychosocial assessments of patients to identify emotional, social and environment factors related to diagnosis, illness, treatment and/or psychosocial assessments including but not limited to crisis intervention, education, advocacy.
Referral management and/or mediation.
Assist with screening identification and intervention in cases involving suspected abuse, neglect, intimate partner violence, exploitation and sexual assault.
Collaborate with and provide education to the interdisciplinary treatment team and the court as needed regarding patients assessed needs, care transition options and patient choice: providing representation of the social work perspective.
Care plan development.
Monitors, evaluates and documents patient progress according to measureable goals described in the individualized patient care plan ensuring plans remain updated based on the patients current needs.
Connect with community and private resources to address gaps in Healthcare.

Minimum requirement:

Licensed Clinical Social Worker
Achieves and maintaintains Non-Violent Crisis Intervention Certification

Constantly works near moving mechanical parts
Constantly exposed to blood and/or body fluids and infection.
Occasionally exposed to toxic or caustic chemicals
Occasionally exposed to risk of radiation
The noise level in the work environment is usually moderate.
