We are looking for full time MDS Coordinator to join our team!
Candidates must possess the following :
Nursing or related degree from an accredited college or university.
Current RN license in the State of New York
Knowledge of MDS / CAAs / CCP process preferably with experience as an RN Manager or MDS / CCP Coordinator.
1 year experience in long term care setting.
PRI Certification.
The following details are some of the responsibilities for the MDS Coordinator :
Evaluates all new admissions, re-admission and bed retention returnees within 24 to 72 hours for Medicare / Medicaid eligibility and / or significant change in status, and informs Director
Ensures the MDS 3.0 is correctly dated with assessment reference dates
Obtains and maintains a current and up to date list of all MDS 3.0 and CCPs due
Evaluates all new admissions, re-admission and bed retention returnees within 24 to 72 hours for Medicare / Medicaid eligibility and / or significant change in status, and informs Director
Ensures the MDS 3.0 is correctly dated with assessment reference dates
