Outreach Health Educator
We are seeking a highly motivated and experienced Outreach Health Educator to join our team on a temporary basis. Outreach Health Educators are patient health educators are frontline public health workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. developing and implementing health education programs to promote healthy lifestyles and prevent diseases. You will work closely with community organizations, schools, and healthcare providers to identify health needs and develop strategies to address them. Your ultimate goal will be to improve the health and well-being of the community we serve.
Minimum Qualifications:
- High School Diploma or equivalency
- Valid NC driver license; acceptable driving record
- 1-3 years clinical/professional experience preferred; equivalent combination of education and/or experience.
- Strong knowledge of health promotion and disease prevention principles
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Responsibilities:
- Understands that possessing a driver license and a clean driving record/ability to be insured as a company driver is a condition of employment.
- Available to work evenings, and weekends as needed.
- Understands their work schedule will be assigned according to the growing season and arrivals of MSFW’s.
- Works without direct supervision, adheres to company policies and notifies the Director immediately of barriers to implementing the job as outlines.
- Communicates and follows instruction from Outreach Care Coordinator and Community Development Director regularly regarding Outreach to Migrant/Seasonal farmworkers, families and other underserved populations in the community.
- Participate in population based projects that enhance retention in care, self-management, treatment, adherence and compliance with accepted health care standards.
- Participate with community, city-wide or statewide resources and/or professional development with other organizations such as, but not limited to North Carolina Farmworker Health, NC Department of Health and Human Services, North Carolina Community Health Center Association, and National Community Health Center Association.
- Participate in assigned community based activities as assigned, including outreach to potential new patients and connecting them to care and services, and will assist with the set up and breakdown of event props. (Table, chair, displays) as applicable.
- Responsible for accurately completing Outreach patient assessments, PRAPARE screening tools, mileage logs, and other record keeping.
- Interprets and translates/speaks Spanish regularly for clinic operations/events.
- Regularly, no less than every other day, completes warm hand-offs i.e. turns in health assessment for billing, scanning, and entry into the E.H.R to Outreach Care Coordinators or other assigned staff. If able, will complete this paperwork themselves if time permits.
- Initiates the patient registration process and correctly completes associated forms, and gathers required documents.
- Explains all appropriate resources for assistance including 340B, Prescription Assistance Program, ICOR, DSS, Lenoir County Transit ridership, DSS Medicaid transportation vouchers, WIC and appointment-based care (PCMH) as opposed to the acute walk-in care; Educates patients to comply with all center requirements for care.
- Follows protocols and utilizes sound judgements to appropriately respond to requests for services. This may include facilitating the scheduling, documenting outcomes, and notifying the patient of the pending appointment.
- Maintains BLS and effectively utilize CPR skills when required
- Regularly communicates with patients, staff, and community organizations.
- Provide basic education, screening, and support and/or facilitate referrals to other health center or community-based resources. Basic patient education will include but is not limited to and assigned as deemed appropriate to the individuals knowledge by the Community Development Director:
- HIV, STD, and other chronic diseases such as Asthma, Diabetes, Hypertension
- Healthy weight management
- Depression
- Domestic Violence
- Tobacco cessation
- Pediatric/Parenting Education
- Health system education and advocacy to assist patients in navigating the healthcare system
- Provides blood pressure checks, blood sugar checks, and performs other simple clinical measurements.
- Provides transportation using a company vehicle, for patients upon request. Follows all transportation policies.
- Maintains a professional appearance, demeanor, and dedication to service.
- Adheres to all federal regulations regarding HIPAA as well as KCHC employee policies.
- Completes all other duties as assigned.

As an Outreach Health Educator, you will utilize your strong communication and interpersonal skills to engage with community members and healthcare providers. You will also use your knowledge of health promotion and disease prevention principles to develop effective health education programs. In addition, you will need to be organized and detail-oriented to maintain accurate records of program activities and outcomes. Finally, your ability to work independently and as part of a team will be essential to your success in this role.
