The Village at Brookwood, located in Burlington, North Carolina, and offers a full continuum of care services for older adults, including independent living, assisted living, skilled nursing and memory care.
Summary
Responsible for assisting in the planning, implementing, and assessing activities for skilled nursing for adults who have dementia, development abilities &/or physical impairments.
Essential Functions
Assist in planning, organizing, and implementing daily activity programming.
Participate in staff meetings to coordinate activity planning.
Lead activities, as appropriate to skills and interests.
Formulate and maintain activity outlines.
Request program supplies/materials, as needed.
Incorporate environmental space in activity options with seasonal and age-appropriate materials and projects.
Assist team to maintain environment, including housekeeping.
Assist with preparation of participant nametags and closet space.
Assist participants with ADL's.
Monitor participants for wandering.
Greet families and participants in AM and/or PM; communicate in a professional manner with family members.
Deliver and receive caregiver messages and route appropriately.
Participate in staff training days, workshops, conferences, and educational opportunities.
Required Knowledge, Skills, Abilities And Traits
Shall have completed a minimum of two years of formal post-secondary education or shall have a high school diploma or GED and a combined minimum five years’ experience and training in services to elderly or adults with disabilities.
Shall have at least two years of work experience in supervision and administration.
Nurse’s aide, home health aide or equivalent training course, or a minimum of one year of experience in caring for impaired adults.
Shall have or be able to obtain CPR/First Aid certification.
Shall have a positive, creative, flexible, and outgoing approach to people.
Must be able to lift 35 pounds frequently and 50 pounds or more occasionally.
