The Health and Wellness Coach is an integral member of the Lifestyle Medicine team, working collaboratively with diverse patient populations to engage them in their health and well-being. The Coach co-leads shared medical appointments and partners with patients to develop individualized action plans that support them in achieving their personal health goals. The Health and Wellness Coach is responsible for facilitating the end-to-end coaching process for both individuals and groups. This role is primarily remote but the Coach will come to our Spokane clinic up to once a month for team meetings and trainings. This is a .5 FTE position, working 20 hours per week.
What you'll do :
- Provide personalized one-on-one 30-minute health coaching sessions focused on the patient's stated health goals.
- Co-develop action plans and SMART goals to support patients in achieving their health objectives while providing ongoing accountability and progress monitoring.
- Guide patients in completing goals within the designated coaching term.
- Understand and consider patients' health conditions in the coaching process, consulting with the clinical care team as needed.
- Connect patients with relevant community resources and employee benefit plan information as appropriate.
- Maintain accurate and up-to-date documentation of all patient interactions within the Electronic Medical Record (EMR) system.
- Conduct outreach calls, including follow-ups to enhance patient engagement.
- Manage an active caseload of Shared Administration and Health and Wellness patients.
- Facilitate group medical visits in collaboration with clinicians for groups of up to 14 patients.
- Update and enhance materials used in shared medical visits.
- Work alongside the Lifestyle Medicine team to conduct outreach and recruit new patients.
- Partner with the clinical care team to gain insights into patient health goals and tailor coaching approaches accordingly.
- Serve as a subject matter expert within the care team on coaching methodologies and behavior change strategies.
- Play an active role in supporting the care team in meeting clinic performance metrics and closing patient care gaps.
- Participate in team meetings and daily huddles to ensure seamless integration of coaching into patient care.
