Our professional interdisciplinary hospice team is comprised of physicians, nurses, social workers, spiritual counselors, bereavement coordinators, and CNAs.
Position:
We are seeking a full-time RN Case Manager Hospice to join our dynamic team for weekdays and shared on-call. This exempt-salaried position is responsible to oversee primary a caseload for hospice patients and provide highly skilled nursing care to them, their families, and caregivers in facilities and/or residences as prescribed by the physician. Compliance with the hospice Medicare Conditions of Participation, Virginia Hospice licensure laws and agency policies and procedures.
The ideal candidate will provide high quality patient nursing care including initial assessment, interventions, preventative and therapeutic nursing procedures, documentation, collaboration with Hospice Interdisciplinary Team (IDT) for plan of care, implement plan of care and evaluate relative to patient nursing needs, ensure comprehensive, responsive goals and interventions, and admissions. Maintain effective communication with patients and their family members and IDT. Supervise direct care staff and coordinate delivery of care services.
This role interacts with a variety of stakeholders including patients, family members, clinical staff, physicians, management, and referral sources (ALF's, SNF's, Senior Communities and Hospital Case Managers).
Duties:
- Obtain essential information to function effectively in hospice setting where the patient/family/caregiver is receiving care.
- Collaborate with the physician(s) to plan and implement the medical and nursing plan of care. Administer medication and treatment as prescribed by the physician in the plan of care. Review nursing component of comprehensive interdisciplinary plan of care with the IDT and participate in the IDT care conference.
- Conduct appropriate patient assessments in accordance with hospice guidelines in an accurate, compassionate, timely manner. Recognize verbal and non-verbal cues, and report patient status changes and needs to other disciplines and appropriate agency staff. Interpret findings, initiate appropriate therapeutic actions, and evaluate results of actions.
- Perform and document all duties as ordered in the electronic medical record including completed skilled nursing notes, and hospice CNA supervisory notes in a timely manner. Ensure all documentation and paperwork, including re-certification, is completed, obtain appropriate signatures, and submit all required documentation per agency policy.
- Anticipate patient needs, demonstrate compassion, respect, professionalism, confidentiality, reliability, excellent verbal and written communication, friendliness, attention to detail, organizational skills, time management, prioritize multiple duties seamlessly, and follow through.
- Meet the medical and emotional needs of the patients, family members, and care givers. Address concerns and provide education on care options.
- Adaptive to changing scenarios and crisis intervention for patients when needed.
- Facilitate patient and family education to promote continuity of care and optimal patient/family outcomes for quality of life as defined by the patient/family/caregiver.
- Follow all health, nursing, sanitary, and infection control policies and maintain established standards of practice.
- Supervise, provide direction, and instruction to hospice CNA team members related to provision of personal care and related support services. Perform hospice CNA supervisory visits at least every two weeks (14 days) per Medicare Conditions of Participation.
- Manage daily patient visits and assignments, provide skilled nursing visits according to visit schedule, and notify management of schedule alteration needs. Confirm with patients or patients’ families for upcoming visits.
- Ensure all care is provided in a respectful manner with patients and families, safeguarding dignity, privacy, and confidentiality.
- Participate in on-call rotation, staff and departmental meetings, hospice IDT meetings, in-services, community programs, and committees as needed.
Requirements:
- Graduate of accredited nursing school.
- Current unencumbered Virginia RN license.
- At least two (2) years of RN experience in a hospice, home health agency, or other community health program. CHPN certification preferred. Bilingual is a plus.
- Current BLS, CPR with valid certificate/card.
- Current COVID vaccine required to support health and safety of patients and families.
- Valid Virginia driver’s license, reliable transportation, and personal auto insurance in accordance with Virginia requirements.
- Experienced and knowledgeable in hospice or similar healthcare setting regarding hospice philosophy of care, appropriate safety, infection control, sanitary principles and procedures, medical terminology, electronic medical records management (HCHB preferred), medical supplies, and equipment.
- Able to successfully deliver and follow verbal and written instructions accurately and in a timely manner.
- Demonstrate excellent leadership and teaching skills with staff providing care.
- Demonstrate engaging and positive demeanor with ability to communicate tactfully and effectively with patients, family members, management, staff, and community members. Model service excellence skills.
- Able to exert a minimum of 50 lbs. of energy in order to assist patients. Able to sit, stand, walk, climb stairs, reach, turn, bend, stoop, crouch, kneel, balance, and maintain body equilibrium to prevent falls, reach over the head, push/pull, and grasp with both hands.
