Job Description
The Memory Care Coordinator is responsible for passionately supporting and promoting a healthy, activity-enriched lifestyle within the Heirloom Memory Care program. Coordinate, and facilitate services, programs and Art of Living Well engagements provided by the staff for the residents within Memory Care, ensuring exemplary service delivered with integrity, dignity and compassion. Responsible for the delivery of quality service by encouraging independence and ensuring each resident’s physical, social, cultural, and intellectual well-being.
Availability: Sunday-Thursday 10am-6pm
Pay: $24-$26/hr
What will I do every day?
- Monitor and evaluate residents regularly and assess levels of care
- Plan, prepare and monitor daily work schedules
- Plan and prepare assignment sheets for resident assistants
- Actively support and facilitate wellness programs that meet the needs of each resident
- Interface with physicians and other care providers
- Monitor all physician orders and ensure they have been followed
- Maintain contact with outside health care services pertaining to the resident’s needs
- Oversee medication control and coordinate with physicians for optimum health care
- Handle emergency situations responsibly and calmly until aid arrives
- Conduct and/or assist with regular staff meetings and daily stand-ups
- Review payroll documentation to minimize missed punches and monitor overtime
What will I need to be successful in this role?
- Basic competency in the use of multiple forms of technology, including: Microsoft Office applications, internet-based and custom applications
- Ability to work a flexible schedule, including weekends and holidays as needed
- Degree or certification in health services, social services or engagement /activities / recreation therapy; or related experience
- One to three years’ experience working in senior care, with experience working with persons living with dementia or related conditions or special groups, or engagement / activities / recreation therapy
Special Requirements/Certifications I may need?
- Must have or be able to obtain the appropriate state license required by law
- CPR Certified
- LVN/LPN if required
