Kitsap Mental Health Services (KMHS) is hiring a Community Hospital Liaison to provide clinical and administrative coordination between community hospitals, KMHS outpatient teams, and the Mobile Crisis Outreach Team. This role ensures continuity of care for individuals transitioning from psychiatric hospitalization or emergency department visits to KMHS outpatient services. The Hospital Liaison also monitors clients discharged on a Least Restrictive Alternative (LRA) who are not actively engaging in KMHS services, ensuring compliance with required treatment. Primary Responsibilities:
- Receive and process hospital discharge summaries, ensuring timely distribution to the appropriate clinical teams.
- Coordinate follow-up appointments with KMHS clinicians, prescribers, and intake teams, ensuring clients receive timely access to care.
- Serve as the primary point of contact for hospital discharge coordinators, providing risk and needs assessments as necessary.
- Facilitate connections to specialized KMHS services, including Residential Services, PACT, and WISe, to ensure seamless transitions.
- Verify insurance coverage before intake appointments and provide alternative community resources for those without coverage.
- Maintain strong relationships with community hospitals and participate in key resource management meetings to ensure coordinated care.
- Document all client interactions and activities in compliance with agency guidelines.
- Monitor clients on LRA status who are not engaged in KMHS services, providing referrals, psychoeducation, and motivational interventions to support participation in required treatment.
- Collaborate with the Mobile Crisis Outreach Team and supervisors to assess client risks and determine the need for extended LRA orders.
- If credentialed, conduct screenings and intakes for clients transitioning from inpatient hospital settings to KMHS services.
Bachelor’s Level Minimum Qualifications:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Experienced (minimum 2 years of job-related experience)
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must meet agency driving requirements, including meeting the minimum age requirement of 21 years old, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver's license in the state of Washington, and having reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration (or eligibility)
LICENSURE: Eligibility for Agency Affiliated Counselor Registration.
Current licensure as a Substance Use Disorder Professional (SUDP) in WA State. Must meet and maintain the requirements of a SUDP Approved Supervisor as defined in WAC 246-811-049 and ability to provide supervision to SUDP/T.
Must maintain a Washington State Department of Health license in good standing.
Must meet and maintain the definition of a Mental Health Professional as defined in RCW 71.05.020.
Master’s Level Minimum Qualifications:
EDUCATION: Master’s Degree in Psychology, social services, or behavioral health.
EXPERIENCE: Experienced: for Certified AACs - five years of direct client care under the supervision of a master’s level mental health professional with an active license; for Master’s Level professionals, 2 years of job related experience in a therapeutic setting.

LICENSURE: Certified Agency Affiliated Counselor, Licensed Agency Affiliated Counselor, Full or Associate Mental Health Counselor, Marriage and Family Therapist, or Independent Clinical Social Worker License.

- Proficiency with MS Word and Excel spreadsheet functions related to tracking insurance authorization, logging of records released to MCO’s and AS-BHO, and tracking LRA “monitoring only” clients.

Mental/Physical Requirements: While performing the job duties, the employee is required to walk, sit, use hands and fingers, reach with arms, talk, or listen. Peripheral vision is also required for this position.
