Crisis Care Coordinator – Crisis Triage Center

Hiring Range: $26.89 - $33.61 Per Hour

Are you an experienced behavioral health professional passionate about helping individuals in crisis? Join Kitsap Mental Health Services as a Crisis Care Coordinator. In this role, you’ll work as part of a dedicated multidisciplinary team to provide essential support, advocacy, and crisis intervention for clients in need. You’ll play a critical role in coordinating comprehensive treatment and discharge plans to connect clients with the right resources and support, ensuring continuity of care throughout their crisis journey.

 

About The Crisis Triage Center:

The voluntary Kitsap County Crisis Triage Center provides short term stabilization and crisis services (up to 5 days) to adults experiencing a psychosocial crisis such as a mental health issue, eviction, domestic violence, substance use, homelessness, and more. Multi-disciplinary teams of Peer Counselors, Treatment Aides, Mental Health Professionals and Recovery Case Managers work together to address each individual’s needs. Staff support the client in developing a collaborative plan to navigate the crisis.


Primary Responsibilities:

  • Provide care coordination and direct services to clients in crisis, including individual and group counseling, crisis intervention, and case management.
  • Advocate for clients’ rights and coordinate with community resources (e.g., DSHS, court systems) to facilitate integrated treatment and support.
  • Collaborate with case managers to connect clients with primary healthcare providers and other essential health services, promoting self-management skills.
  • Work closely with designated crisis responders and mental health professionals to provide crisis intervention and implement discharge plans tailored to each client’s needs.
  • Document client progress and ensure accurate and timely record-keeping, maintaining confidentiality and compliance with agency standards.
  • Educate clients on substance use issues and coordinate treatment with internal and external providers.
  • Develop individualized treatment goals and review treatment plans collaboratively with clients, mental health professionals, and team members.

 

Minimum Qualifications:

EDUCATION: Bachelor’s Degree in Psychology, Social Services, or Behavioral Health Field

EXPERIENCE: Specialist (minimum 5 years of relevant experience)

 

5 years of relevant work experience in crisis outreach, behavioral health treatment court, high-intensity outpatient behavioral health treatment (PACT, ACT, etc.), or similar settings. The scope of practice includes counseling as defined under RCW 18.19.020 and the ability to make provisional assessments and diagnoses, which must be reviewed by a clinical supervisor who is able to independently provide mental health assessments and diagnoses.

 

LICENSURE: Eligibility for Certified Agency Affiliated Counselor Registration

 

Preferred Qualifications:

EXPERIENCE: Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
