Summary
The primary role of the Nurse Delegated Coordinator is to provide support to clients on their caseload requiring nurse delegated and other medically intensive tasks. Assist clients to achieve their goals in accordance with the residential guidelines. KIM is looking for qualified candidates to fill Saturday - Sunday shifts. Shifts options are as follows :
Shift One : Saturday-Sunday (12 : 00am-8 : 00am)
Shift Two : Saturday- Sunday (4 : 00pm-12 : 00am)
Note : Both shifts can be combined for weekend double shifts.
Salary
20.00- $21.00 ($22.00-$23.00 for weekend shifts) per hour
Essential Duties and Responsibilities
- Provide Trauma-Informed, Crisis Intervention and Prevention Intervention Support.
- Support clients in achieving and identifying behavioral goals.
- Document client behaviors appropriately.
- Facilitate coordination of managed care.
- Acknowledge and advocate for the needs, wants, and rights of clients with dignity and respect.
- Identify areas of change that clients on their caseload would like to achieve.
- Coordinate individualized programs for clients and document activity thoroughly.
- Accompany clients on outings and activities.
- Transport clients to and from all agency activities, and / or arrange transportation services as appropriate.
- Assist with client money management such as petty cash, receipts, checkbook and EBT’s.
- Supervise clients to ensure adequate safety.
- Keep all hazardous materials, medications, etc. in the designated secured location.
- Implement a safe and clean environment in which our clients live by doing households ADLS (i.e., chores, meals, bathing, hygiene, removal of garbage, laundry, etc.).
- Report promptly instances of injuries, behaviors, abuse, and safety hazards, etc. to management, following the Chain of Command.
- Ensure adequate health care for each client.
- Assist clients with mobility needs, transferring them from wheelchair to bed.
- Monitoring vitals (if applicable) and client behavior.
- Turning or adjusting clients in bed to prevent bedsores and / or other discomfort.
- Ensure clients have access to regular meals.
- Accompany clients to health appointments and provide appropriate documentation for each appointment to management in a timely manner.
- Communicate with medical offices, pharmacies, and the nurse delegator.
- Administer medications to clients and document accordingly, including nurse delegated tasks.
- Assist client with daily hygiene as outlined in care plans, including personal care such as brushing teeth, bathing, hair washing, toileting, etc.
