General Position Summary

The position provides targeted case management services to individuals experiencing Intellectual/Developmental Disabilities (I/DD) by identifying individual needs and developing plans to access available services and resources.

Essential Functions

The following may not be all inclusive and other duties may be assigned. 

Assess and periodically reassess an eligible individual to determine service needs. Develop and periodically revise an ISP or Annual Plan based on information collected through an assessment or reassessment that specifies the desired outcomes, goals, and actions to address the medical, employment, social, educational, and other services needed by the eligible individual. Access available services, including referral and related activities to help an individual obtain needed services. Monitor follow-up activities to ensure that the ISP or Annual Plan is effectively implemented and addresses the needs.

Assist eligible individuals/families in developing and implementing a support system for the specific needs of each eligible individual resulting from of a Level of Care assessment and corresponding assessment of need.

Utilize choice advising to support eligible individuals in their selection of service, provider, and setting options, including non-disability specific settings.

Develop appropriate plans per timelines and monitor for effectiveness and Medicaid standards. Check Medicaid authorized expenditures and monitor service hours through Plan of Care (POC).

Deliver services using person-centered practices to assist individuals in accessing needed medical, employment, social, educational, and other services. Services include but are not limited to assessment and periodic reassessment of individual needs and preferences; development and periodic revision of the Individual Support Plan; Referral and related activities; monitoring; and follow-up activities.

Identify, strengthen, expand, and where required, supplement private, public, formal, and informal support available to individuals with intellectual or developmental disabilities. Encourage the exercising of self-determination in the design and direction of the individual receiving services.

Authorize any developmental disabilities service chosen by the individual, or as applicable the legal or designated representative of the individual.

Develop and maintain effective working relationships with individuals, families, providers and community partners; offering excellent personal centered services.

Review individual’s satisfaction with services and supports.

Represent DD/ID on community boards/councils as directed by KCDDS Leadership Team.

Coordinate with other state, public, and private agencies regarding services to individuals.

Assure an individual has all initial assessments and reviews them according to state guidelines.

Maintain working knowledge of laws, rules, policies, contracts, transmittals, and any other relevant standards or regulations.

Maintain a caseload of individuals in various service elements as assigned.

Participate in transition planning by attending IEP meetings or other transition planning meetings.

Delivery of case management services to individuals as described in OAR chapter 411, division 415.

Assist the Director in monitoring the quality of services delivered and identify existing and insufficient service resources or options within the County.

Continue to enhance knowledge, as well as maintain a basic understanding of the skills, knowledge, and responsibilities necessary to perform the position. Complete a minimum of 20 hours of I/DD training every year.

Establish and maintain appropriate professional demeanor and boundaries at all times.

Assure and maintain the rights to confidentiality for all individuals in services and their families.

Fulfill responsibility as a mandatory abuse reporter.

Supervisory Responsibilities

This position has no supervisory responsibilities.

Qualifications

Participate in a core competency training sequence approved by the Oregon Office of Developmental Disabilities Services (ODDS) and Klamath County Developmental Disabilities Services (KCDDS) Department.

Education, Certification, & Experience

Must have knowledge of the public service system for developmental disabilities services in Oregon.

  1. A bachelor's degree in behavioral science, social science, or a closely related field;
  2. A bachelor’s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or
  3. An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing

Skills & Abilities

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.  Ability to write reports, business correspondence, and procedure manuals.  Ability to effectively present information and respond to questions from groups of managers, individuals, and the general public.

Mathematical Skills

Ability to add and subtract two digit numbers and to multiply and divide with 10's and 100's.  Ability to perform these operations using units of American money and weight measurement, volume, and distance.

Reasoning Ability

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.

Computer Skills

To perform this job successfully, an individual should have knowledge of Word Processing software.

While performing the duties of this Job, the employee is frequently required to sit and use hands to finger, handle, or feel.  The employee is occasionally required to stand; walk; reach with hands and arms and talk or hear. The employee must frequently lift and/or move up to 10 pounds.  Specific vision abilities required by this job include close vision, peripheral vision and ability to adjust focus.

The noise level in the work environment is usually moderate.

Work Schedule

The normal work schedule is 40 hours per week; exact schedules is assigned by supervisor in conjunction with the KCDDS Executive Leadership Team.  It is the attendance standard of the employer that all employees be present and on time each shift they are scheduled to work.  Full or partial absence during any scheduled shift, arriving late, or leaving early, for whatever reason, may result in disciplinary action up to and including termination of employment.  Time off may be approved if allowed by applicable policies covering illness, injury, or other personal emergency.
