KHP is dedicated to providing high-quality, patient-centered care. We are seeking a compassionate and skilled Behavioral Health Case Manager to join our team and contribute to the health and well-being of our diverse patient population.
The Behavioral Health Case Manager provides supportive services which promote the clients ability to function independently, improve the quality of their lives and/or improve ability to benefit from other supportive services (i.e. counseling, medication management, etc.) These may include assistance with coordination with other treatment providers, OHP, SSI, housing, vocation, medication, transportation, family meetings, skills building, etc.
Key Responsibilities
- Is knowledgeable about and assists clients in accessing local or regional resources (i.e. housing, job opportunities, medications/prescriptions, food, Medicaid/insurance, etc.)
- Responds quickly to requests made by supervisor or therapists.
- Works cooperatively with fellow treatment team members.
- Works respectfully and effectively with clients and their family members.
- Works cooperatively with outside agencies or schools.
- Responds appropriately to high risk or crisis situations.
- Monitors safety of clients on their caseload. Reports all cases of suspected abuse or neglect to the proper authorities.
- Assists clients with intake paperwork or other paperwork as needed.
- Assists clients in obtaining and/or managing their medications.
- Provides or coordinates transportation for therapy appointments.
- Performs other duties as assigned.
Desired Knowledge, Skills, and Abilities:
- QMHA or Certified MH Peer Support Specialist or Certified Community Health Worker (CHW) with some MH experience (Required).
- Bachelor’s degree in Social Sciences with two years’ experience working with mentally ill adults and/or children and families or 3 years combined work, education, training, and/or experience.
- Must have valid Oregon driver’s license, insurance, and own transportation.
- Bilingual preferred but not required.
- Effective written and verbal communication skills.
- Must be detail oriented, organized, and work well independently and as part of a team.
