KHP is committed to providing high-quality, patient-centered care. We are seeking a compassionate and skilled Family Nurse Practitioner to deliver primary care and health promotion services to our diverse patient population.
Position Summary: Under the direction of professional medical providers, the Family Nurse Practitioner will perform comprehensive primary care services. The ideal candidate will provide quality, cost-effective care, ensuring patient confidentiality and maintaining timely documentation in accordance with clinic protocols.
Key Responsibilities:
Health Promotion Services:
- Conduct school physicals, immunization clinics, well-child and well-woman clinics, adult focus clinics, blood pressure screening clinics, nutritional counseling, and family planning counseling.
- Provide prenatal care when applicable.
Patient Care:
- Perform, order, and interpret laboratory tests, x-rays, and other diagnostic tests.
- Obtain and record patient histories and physical examinations.
- Evaluate and treat minor illnesses and injuries.
- Monitor and treat chronic diseases.
- Perform surgical and other medical procedures within competency.
Health Education:
- Conduct health education activities for individuals and groups.
- Promote health by advising patients on diet, hygiene, and disease prevention methods.
Consultation and Referrals:
- Obtain consultation or referrals for services not provided by the clinic.
- Collaborate with healthcare professionals to ensure comprehensive care.
Additional Duties:
- Provide additional services as agreed upon with the Medical Director and Executive Director.
- Participate in quality improvement activities.

Qualifications:
- Completion of MSN and Family Nurse Practitioner program.
- Current, valid license to practice medicine in the State of Oregon, including DEA license and CPR certification.
- Current BLS certificate.
- Relevant and substantial knowledge of health center licensing and operations, particularly within an FQHC environment.
- Ability to work effectively both independently and as part of a team.
- Commitment to maintaining the highest level of confidentiality.
- Sensitivity to the needs of a culturally diverse patient and employee population.
- Willingness and ability to work some evenings and weekends as needed.
- Pleasant, professional attitude toward patients, customers, co-workers, and supervisors.
- Strong teamwork skills.
Physical Demands and Accommodations:
- Must be able to lift and/or move up to 25 pounds occasionally.
- Work Monday through Friday, 8:00 a.m. – 5:00 p.m., with occasional extended hours and Saturdays.
- Regularly required to walk, talk, bend, lift, carry, write, and hear.
- Frequently required to stand, walk, sit, and use hands to finger, handle, or feel.
- Specific vision abilities required include close vision, distance vision, color vision, depth perception, and ability to adjust focus.
