Case Manager & Community Liaison Position – Lake County only
This multi-faceted, multi-responsibility position is critical to the success of the Klamath and Lake Counties Council on Aging (KLCCOA) programs throughout the Lake County region of Oregon. This position will case manage the home delivered meals program and the Oregon Project Independence program (in-home care); assist in the management of the Aging and Disabilities Resource Connection (ADRC), Options
Counseling, Family Caregiver Respite and home modification programs. This position is also a community liaison between KLCCOA services and other agencies that serve the aging population.
Job Duties:
Case Management
The duties of a Case Manager include but are not limited to:
Information and assistance and referral to clients and public regarding local programs
Outreach to public and seniors including low-income, isolated, minorities
Computer use for data input of client services
Assessment of needs and eligibility for Oregon Project Independence, Meals on Wheels, and Family Caregiver Support programs
Work with providers of in-home service to maintain care plan
Provide periodic plan review and reassessment as prescribed by the policy of programs
Assist clients who are unable to communicate clearly to ascertain needs or provide support
Monitor care/actions plans for appropriate services for clients as individual situations change
Provide advocacy on behalf of clients as necessary, representing client’s interests for medical care and providing assistance and follow-up on consumer complaints
Participate and facilitate community awareness with other local service providers
Work effectively with other social agencies to partner for care needs of client
Maintain knowledge of functions and eligibility of public and private agencies and institutions providing social support service to the elderly
Complete monthly reports in a timely fashion
Maintain records for all programs and persons receiving service from those programs
Provide narration, related to events in sequence involving details, observations of person, place and events
Provide Options Counseling with a needs assessment and an action plan for self-determination of client, including follow-up
Travel to trainings as needed or recommended to enhance job knowledge
Home visits (including traveling in adverse weather conditions)
Provide own transportation and insurance (mileage reimbursed)
Options Counseling
Options Counseling is a core ADRC service that supports informed long-term care decision making through assistance provided to individuals and families. The core competencies of Options Counseling are:
Determination of the need for options counseling
Assessment of needs, values, and preferences
Understand public and private sector resources
Facilitation of self-direction
Follow-up
Community Liaison
The duties of a Community Liaison include but are not limited to:
Interaction with other community agencies/programs that serve seniors that are 60 years old and/or disabled
Community presentations for education of KLCCOA programs
Promote and coordination of health promotion programs
The position prefers a bachelor’s degree with social services, case management, or information and referral experience or the equivalent combination of education, training, and experience. Applicant needs to have good communication skills, be reliable and completes work in a timely and accurate manner.
The position is full-time and requires successful completion of a background check. Klamath-Lake County

Full-Time (1.0 FTE) $17.50 per hour for 3-month probation period. $18.00 per hour after probation period.
Benefit stipend $150 per month
Phone stipend: $30.00 a month
Mileage: IRS annual rate per mile
