GENERAL SUMMARY :
The Palliative Care Nurse provides Palliative Care Services to patients in the clinic, hospital, and community settings. The Nurse must be able to think and work independently, creatively, and productively with an interdisciplinary team of healthcare workers. This work will be complex - both broadly conceptual and, at times, tedious and detailed - requiring a mature, holistic approach to be successful. The Nurse is primarily responsible for coordinating and expediting care for palliative care patients; effectively communicating with patients, providers, and community support organizations to coordinate and facilitate a comprehensive plan of care; and facilitating a shared goal model within and across settings to achieve coordinated, high-quality care that is patient / family centered. The Nurse demonstrates excellence in customer service during interactions with patients, families, and other customers.

Minimum Education, Training & Experience (includes licenses or certifications) :

Education / Licensure :
- Must be an accredited nursing school graduate with an active / current RN license in Washington State.
- Current BLS certification required.
- Valid Washington State Driver's License.
Experience :
At least one (1) year of recent full time experience providing direct patient care in an outpatient or acute care medical setting preferred, as well as one (1) year End of life Care experienceSkills / Knowledge / Abilities :
Ability to work independently using sound judgment and critical thinking skillsDemonstrates a thorough working knowledge of nursing theory, techniques, principles and practice in order to safely and competently care for the patient.Knowledge of chronic care management, advanced care planning, transitions of care and remote patient monitoring strongly preferredKnowledge and awareness of local and regional resourcesAbility to assess the patient's unique needs and generate resourceful ideas / recommendations.Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition and ability to cope with both physical and emotional demands of the job.Maintains a calm and composed manner in emergency / stressful situations, taking appropriate action as necessary.Ability to listen and accept advice from othersKeen attention to detail and high degree of accuracy; risk of error could result in significant harm or loss.Proficiency using everyday technology (e.g., email, data retrieval, voicemail, and internet / intranet) as well as ability to effectively utilize the electronic medical recordExcellent organizational and multi-tasking skills with ability to anticipate tasks and prioritize assignments.Ability to read, write, speak and understand English.POSITION SPECIFIC RESPONSIBILITIES
Upload health information into patient charts, take vital signs, weights, physical assessments, INR and Blood Sugar Monitoring, reconcile medications, and other nursing services as neededPlace follow-up calls to patients, families, providers, pain clinic, home health coordinators, and all other entities as neededAttend monthly Palliative Care Meetings and conference callsAssist with Palliative Care Community events and trainingsMeet with patients and families in their homes as neededHelp assist with developing a patient centered care plan with patient's wishes / goals, along with interventionsBe an advocate for the patient and family suffering from a serious life limiting illnessSchedule Advance Care Planning Meetings as needed, and fill out required documentationDeploy remote patient monitoring technology and monitor utilizationConnect patients / families with local and regional resources (e.g. Meals on Wheels, Senior Services, Mt.
