Responsibilities
- Provide direct patient care in various healthcare settings such as hospitals, assisted living facilities, and long-term care facilities
- Administer medications and treatments as prescribed by healthcare providers
- Collaborate with the healthcare team to develop and implement patient care plans
- Conduct patient assessments and assist with discharge planning
- Utilize electronic health record systems like Epic, eClinicalWorks for documentation
- Assist with occupational health programs and services
- Coordinate durable medical equipment (DME) needs for patients
Skills
- Proficient in patient care techniques and procedures
- Experience with inpatient care and long-term care facilities
- Knowledge of discharge planning processes
