Responsibilities:
- Telephonic support: Provide expert clinical guidance and education to nurses, therapists, and aides via phone consultations, addressing patient needs and concerns, and offering timely interventions.
- Virtual visits: Conduct comprehensive virtual assessments and follow-up visits with patients, utilizing technology to monitor progress, manage care plans, and document interventions.
- Quality control: Analyze patient data and clinical documentation to identify areas for improvement, implement quality initiatives, and ensure compliance with regulations and best practices. Perform outreach calls to patients and family members to assess quality and satisfaction of services.
- Intake bridge: Collaborate with the intake department to ensure smooth transitions for new patients, reviewing referrals, assessing needs, and developing initial care plans.
- Education and mentorship: Provide ongoing education and mentorship to staff on clinical best practices, new technologies, and evidence-based interventions within their respective disciplines.
- Participation in clinical team meetings: Actively participate in interdisciplinary team meetings to discuss patient care, address challenges, and develop collaborative care plans.
- Data analysis and reporting: Analyze clinical data and prepare reports to identify trends, measure outcomes, and support continuous quality improvement efforts.
- Work collaboratively with Home Health Director, Clinical Manager and Director of Quality and Compliance to promote outstanding quality of care
- Perform in person visits on prn basis
- Participate in Administrator on Call schedule
Qualifications
Qualifications:
- Current licensure as a LPN or Registered Nurse (RN) in Washington state.
- 5 years nursing experience including at least 2 years of HH
- Strong clinical skills and expertise in managing complex patient conditions.
- Advanced communication and interpersonal skills, with ability to effectively build rapport and collaborate with diverse stakeholders.
- Proficiency in telehealth technologies (preferred) and electronic medical records (EMRs).
- Excellent analytical and problem-solving skills, with ability to identify and address quality issues.
- Strong leadership and mentorship abilities.
- Demonstrated commitment to patient-centered care and continuous quality improvement.
