We are currently hiring for the position of On-Call After-Hours Hospice Visit Help RN or LPN, which includes competitive pay with on-call differential. And a $5,000 sign on bonus!
The On-Call After-Hours Hospice Visit Help RN or LPN plays a crucial role in the care and comfort of our hospice patients. The Hospice Visit RN or LPN will be responsible for responding to after-hours patient needs in support of symptom management, psychosocial support, and teaching and guidance for hospice patients and their families. The right person for this position will have nursing experience in symptom management. They will have strong assessment, critical thinking, and communication skills. Additionally, should be comfortable working autonomously and among a care team, are adaptable, empathetic, takes initiative, possess a positive attitude and a passion for working with patients at end of life.
Position Location – Supporting patients who live within King County in various care settings.
Shift – 7 Days on/ 7 Days off on-call schedule between 7pm-8:30am
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
- Respond to after-hours Hospice patient visit needs as needed.
- Serve as back-up Triage RN to support vacation and other coverage needs.
- Conduct urgent after-hours hospice admissions as needed.
- Provides professional nursing care by utilizing all elements of the nursing process.
- Provides skilled nursing care focusing on comfort, pain control and symptom management.
- Provides education and guidance to the patient and family regarding diagnosis, medications, treatments and progress.
- Collaborates with Primary Triage RN, Hospice Physician and Administrator On-Call to support patients in complex care situations.
- Maintains clear and effective communication with patients, families and healthcare professionals in support of patient care.
- Documents care collaboration and patient care provided in the electronic medical record before end of shift.
- Collaborates with vendors to obtain DME, supplies and medications when needed after-hours.
- Engage in care coordination and education with other professionals in the hospital or community residential settings.
- Performs other job-related duties as assigned.
Qualifications
- Has a passion and interest in working in Hospice care.
- Graduate of an accredited nursing program.
- Has three years nursing experience. Community, Home Health, Hospice, Oncology, Medical/Surgical experience is preferred.
- Currently licensed as a RN or LPN in the state or Multistate nurse licensure
- Complies with accepted professional standards and practice.
- Holds a comprehensive knowledge of medication, treatment and therapies for pain and symptom management.
- Demonstrates good verbal and written, communication and organizational skills
- Posses and maintains CPR certification
- Must be a licensed driver with reliable transportation and ability to travel within our service area. Must maintain valid auto insurance that meets state and organizational requirements.
Salary : $45 - $60
