We're on the lookout for a passionate and dedicated Weekends Part Time LPN or RN MDS Coordinator to join our team! We're offering a $2500 Sign-On Bonus!
Pay Scales:
- Licensed Practical Nurse (LPN): $42.00 to $50.00
- Registered Nurse (RN): $48.00 to $55.00

Job Responsibilities:
- Coordinate and Complete MDS Assessments: Ensure accurate and timely completion of the Minimum Data Set (MDS) in line with federal and state regulations.
- Develop Care Plans: Create comprehensive care plans based on patient assessments.
- Effective Communication: Liaise with Direct Care Staff, Physicians, and Families to share observations and updates.
- Medicare Compliance: Ensure all Medicare program requirements are met.
- Select ARD: Choose the correct Assessment Reference Date (ARD) and reasons for assessment to capture all utilized resources for appropriate Medicare/Medicaid payment.
- RAI Process Updates: Disseminate new or updated materials related to the Resident Assessment Instrument (RAI) process.
- Electronic Submissions: Ensure timely and valid electronic submission of all MDSs.
- Validation Reports: Review and follow up on validation reports.
- Staff Training: Work with the Staff Development Coordinator to ensure all licensed nurses and new interdisciplinary team members are trained on the RAI process, and CNAs receive ongoing training on ADL coding.
Qualifications
What we're looking for and what the State makes us look for:
Active license to practice as a Nurse (RN Preferred or LPN) in the State of WA
Strong clinical assessment skills
Ability to make independent decisions on a regular basis
Effective interpersonal skills and the ability to work with an interdisciplinary team
Intermediate computer skills
Proven written and oral communication skills
Proven decision making and analytical skills
and a wonderful sense of fun, teamwork and compassion!
And icing on the cake is:
Minimum two years of clinical experience in a healthcare setting
Minimum of one year of experience in a long term care setting
Prior experience as an MDS Coordinator preferred
MDS 3.0 System experience
