Your Role: Leading Patient-Centered Care
As the Director of Care Management Services, you'll be
instrumental in optimizing patient flow and ensuring seamless transitions
throughout the hospital continuum. You'll lead and develop our Care Management
program, overseeing Case Management, Social Work, Utilization Management, and
post-acute services coordination. Your expertise will enhance staff roles,
streamline care progression, and foster strong physician collaboration.
Key Responsibilities:
- Strategic
Leadership: Develop and implement innovative programs to improve care
facilitation and patient movement.
- Team
Management: Lead and mentor a multidisciplinary team, fostering a
collaborative and efficient environment.
- Program
Development: Enhance the Hospital Case Management program, defining
clear staff roles and processes.
- Utilization
Management: Ensure efficient resource utilization and optimize patient
length of stay.
- Post-Acute
Care Coordination: Facilitate smooth transitions to post-acute care
settings.
- Collaboration: Work closely with physicians and other healthcare professionals to ensure
coordinated patient care.
- Patient

Requirements
Required Qualifications:
- Current
Montana State RN License.
- Minimum
of five years of progressive case management experience in a healthcare
setting.
- Proven

Preferred Qualifications:

degree in a related field.
- Three
to five years of supervisory experience.
- Formal
case management training/certification.
