Immediate contract RN Hospice Case Manager opportunity servicing patients in the Bloomsburg, PA area.
Pay Rate:
- $55.00/hr
- $.49 mileage reimbursement
Work Schedule:
- 8:00am - 5:00pm (Monday to Friday)
- 40 hours per week
- Covering patients in the following areas: Lycoming, Synder, Union, Northumberland Counties (Williamsport to Sunbury area).
Job Details
- Assess patient needs and develop individualized care plans based on clinical evaluations.
- Coordinate with interdisciplinary teams to facilitate effective communication and collaboration in patient care.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and resources to patients and families regarding treatment options, discharge planning, and follow-up care.
- Advocate for patients' needs within the healthcare system to ensure they receive appropriate services.
- Document all patient interactions, assessments, and care plans accurately in electronic health records.
Qualifications
- Currently licensed Registered Nurse
- Must have at least 12 months of hospice in home settings experience within the past 3 years.
- HCHB and OASIS experience preferred.
- Current CPR certification
