As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
Schedule : Monday- Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care. No scheduled evening / weekend responsibilities outside of phone On-Call

On-Call : Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume -
Requirements :
- Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
- Unrestricted DEA licensure and prescribe authority (per state practice regulation).
- At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and / or pursue OMA APC certification. - You have an active, unrestricted license to practice in Massachusetts, or willingness to obtain. Willingness to obtain additional licensures if needed based upon business needs.
- You have excellent communication and interpersonal skills.

- You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals.

- You are excited to work with a collaborative team of physicians, APC's, registered dietitians, medical assistants, health coaches, and other staff.
