JOB SUMMARY
The Licensed Practical is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments and working collaboratively with the members of the team to help meet positive patient care outcomes. Adheres to organizational policies and procedures, regulatory accrediting body requirements, and professional practice standards, scope of practice.
LPN Responsibilities:
- Provides direct patient care as defined in Ohio’s Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurately and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT) to the registered nurse.
- Completes charting on each patient by the beginning of each work day or within 24 hours of visit.
- Uses equipment and supplies effectively and efficiently.
- Documents all aspects of patient care, maintains patient records and care plans in a timely manner.
- Performs other duties as assigned by the registered nurse.
- Participates in on-call schedule, carries on—call pager and is available by phone during normal business hours.
- Maintains self-control under stressful situations and workload pressures without exhibiting negative behaviors. .
- Demonstrates knowledge of disaster/emergency procedures and responds appropriately.
- Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital, division, and unit standards of care and policies and procedures.
- Utilizes measures to promote and maintain patient, visitor and personnel safety.
- Maintains patient, employee, physician, and organization confidentiality; respects the rights, privacy, and property of others.
- Demonstrates the appropriate use of infection control principles and personal protection equipment.
- Supports the mission, values, and vision of the organization.
- Demonstrates a positive attitude and takes the initiative to implement changes.
ADDITIONAL RESPONSIBILITIES
- Performs tasks that are supportive in nature to the essential functions of the job.
- Participates in staff meetings, educational programs, committees, QI activities and mandatory in-services.

- Assists with control of costs through the judicious use of human and material resources.
- Other duties as assigned.
EDUCATION AND WORK EXPERIENCE
- Graduate of an accredited practical nursing program.
- Current licensure as a licensed practice nurse to practice nursing in the State of Ohio.
- Three years nursing experience. Community/home health or medical/surgical experience is preferred.
- Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
- Valid Ohio’s Driver License and maintain motor vehicle insurance
KNOWLEDGE AND SKILLS
- Current knowledge of the nursing role and scope of practice as defined by the State of Ohio Nurse Practice Act and the Ohio Board of Nursing.
- Current knowledge of American Nurses Association (ANA) Code of Ethics.
- Current knowledge of patient counseling, interviewing, habilitation techniques and procedures.
- Able to utilize the nursing process.
- Current knowledge of community resources and services.
- Current knowledge of current trends and demonstration of standards of care.
- Demonstration of effective problem solving, decision-making, interpersonal, and teamwork skills.
- Demonstration of effective verbal and written communication.
- Demonstration of positive customer service.
