ALCOHOL DRUG CASE MANAGER
Purpose of Job
The purpose of this classification is to assist the Recovery Court (RC) and Veterans Treatment Court (VTC) Teams. Under the general supervision of the Recovery Court Program Director, the position is responsible for providing assessments, monitoring, planning, linkage, and advocacy for the most appropriate services to individuals enrolled in the RC/VTC Court Programs. The Case Manager collaborates with probation, treatment providers, and treatment court team members to coordinate programs and activities for court participants. The case manager must also attend Specialized Court staffing and dockets weekly.
Essential Functions
The RC & VTC Case Manager is responsible for the following essential functions:
- Assist with conducting initial screening of potential participants. Review evidence-based assessments and prepare solution-focused Case Management Plans.
- Link program participants with a continuum of care, including primary health services and public health services, if applicable.
- Complete detailed participant progress reports/notes. Continual monitoring and case management plan modification for phase progression.
- Teach, role model, and coach how to use resources and navigate or exercise skills needed for the management of alcohol and/or substance abuse and hold participants accountable for actions.
- Develop relationships and linkages with treatment providers, harm reduction service providers, primary healthcare providers, transitional housing providers, and other community agencies.
- Provide statistics as requested by administration.
- Meet with assigned participants in Phases 1-5 as outlined in the RC/VTC Program.
- Monitor participant's activities, work status, and living arrangements.
- Conducts urinalysis testing on participants as requested. Meets with participants in jail, treatment providers, homes, court, and other places, as necessary.
- Attend all RC/VTC Staffing Sessions.
- Serve as a liaison between the court, law enforcement agencies, treatment provider partners, community correction/probation, and the community.
Additional Job Functions
- May require occasional travel and on-call duties.
- Assist with housing stabilization, if applicable.
- Participate and lead continuous program improvement initiatives.
- Participate in research and program evaluation.
- Respond to and report immediately if called upon by local, regional, or central office supervisors as part of the Knox County Health Department's coordinated emergency response.
- Ensure regular, punctual attendance during scheduled work hours.
- Perform other duties as required.
Minimum Training & Qualifications
- A bachelor’s degree is preferred in social services, public or community health, criminal justice, public administration, or a related field.
- Minimum of three (3) years of direct work experience in case management in a probation, law enforcement, or substance use treatment position.
- Must possess and maintain a valid TN driver’s license.
- Any equivalent combination of education, training, and experience that provides the requisite knowledge, skills, and abilities may be considered for employment.
Specialized Knowledge
- Knowledge of applicable TN State statutes, rules, orders, and procedures.
- Knowledge of applicable Federal rules and requirements.
