Koinonia Family Services is seeking a dedicated and compassionate Case Manager to join our team for adolescent residential therapeutic programs.
The ideal candidate will work with at-risk youth aged 12-18 who require residential treatment due to early childhood trauma, mental health issues, or substance abuse. The Case Manager plays a critical role in ensuring the well-being and support of these youth, including:
- Managing medication and insurance eligibility
- Facilitating psychiatric and medical appointments
- Transporting youth to needed appointments
- Collaborating with the treatment team regarding medical needs
The Case Manager serves as a positive adult role model and must be able to work productively under minimal supervision, with flexibility to meet work requirements.
