Koinonia Family Services is seeking a part-time Wraparound Facilitator to work in its Post-Adoption Wraparound Program in the Tehachapi area. The program supports families who have adopted children or youth from the foster care system and are facing challenges related to the child’s or youth’s behavior and mental health. The Wraparound Facilitator oversees wraparound services for up to five families, including initiating engagement, forming teams, developing safety and care plans with a strength-based approach. The Facilitator leads with wraparound values, demonstrates interpersonal skills, understands public service systems, builds strong team connections, respects diverse perspectives for success, and provides direct support consistent with the wraparound plan.

This is a part-time position, offering approximately 20-25 hours per week, with the potential to transition to a full-time role as the program grows.
Key Responsibilities:
Under the leadership of the Wraparound Supervisor, you will provide direct services to families and youth, including:
- Initiates the engagement process by understanding the family's history and current situation through their stories, identifying their needs, strengths, trauma, and key relationships
- Explains the post-adoption wraparound process to the family, covering its values and principles, emphasizing the significance of the family's input, choices, and preferences
- Assists parents and youth in identifying and inviting potential Child and Family Team (CFT) members, orienting them to the wraparound process and their roles, and facilitates the CFT meeting
- Develops and facilitates individualized, strength-based, and culturally sensitive care plans with individualized strategies that include supportive linkages
- Provide direct support in homes and communities, addressing the unique needs of youth and families
- Coordinate with team members, community resources, and educational services to ensure comprehensive support
- Maintain meticulous records, ensuring all interventions and plans are well-documented
Qualifications:
- A Bachelor's degree in a behavioral-science field or higher is required
- 2 years of case management experience preferred
- Prior experience in post-adoption wraparound highly preferred
- At least one year of experience working with high-risk youth or families, or comparable lived experience in foster care or adoption settings
- Exceptional interpersonal skills, with a commitment to culturally competent care
- Experience in child welfare, juvenile justice, and crisis intervention
- Strong commitment to cultural competency and ethical practices
- Passion for ongoing professional development
- Valid driver’s license and reliable transportation are required

Qualified candidates must complete a criminal background clearance through the California Department of Justice and FBI to ensure eligibility for employment, as well as a pre-employment physical and TB test. This position requires a valid driver’s license, auto insurance and reliable transportation as driving is an essential job function. Wraparound Facilitators are responsible for tasks such as visiting client homes, attending in-person meetings, and engaging with community partners, often in locations not easily accessible by alternative transportation methods. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the youth and families we serve.
