- Provide therapeutic services to patients, assessing and tracking their stages of change and recovery
- Implement client-centered and family-centered approaches to care with goal-specific interventions
- Conduct initial assessments and collaborate with patients to develop personalized treatment plans
- Build and maintain strong, supportive relationships with patients to meet their evolving needs throughout treatment
- Coordinate care with family members, referral sources, and other service providers
- Monitor treatment progress and participate in collaborative treatment team reviews
- Ensure all documentation is completed in accordance with Medicaid, Medicare, and CARF standards
- Engage in case management duties, including discharge planning, treatment planning, and referrals
- Participate in team meetings, in-service training, and supervision as required
- Master’s degree in Social Work, Counseling Psychology, or a clinically related field
- Active, in-good-standing Maryland licensure (e.g., LGPC, LCPC, LMSW, LCSW-C, LGSW, LCSW)
- Previous experience with adults, substance use, or co-occurring disorders is a plus
- Proficiency in using electronic health record systems (EHR)
- Strong communication skills, with the ability to engage effectively with clients, families, and colleagues
- Commitment to ongoing professional development and keeping up-to-date with the latest practices in mental health treatment
