Responsibilities and duties include but are not limited to :
Assesses the full scope of patient needs using the nursing process on a continuous basisObserves signs and symptoms and reports changes in the patient’s condition to the physician and / or other appropriate health care professionalsProvides teaching to client and client family membersDelegates tasks to other Kore Cares personnel in accordance with the South Dakota Nurse Practice ActUses knowledge and judgement needed to meet nursing needs of the patientNursing tasks may include but are not limited to : wound dressing, medication set up, monitor medication compliance, assess vital signs, assess skin condition, assess nutritional status, assess mobility, assess usage of glucometer, toe / nail care, catheter care, assess health, provide teaching / health education, communicate with physician and pharmacist, blood drawsQualifications :
2 years of nursing experience, preferredMust have active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of SDMust be able to physically perform the duties of the positionMust be able to read, write, understand and speak EnglishThis position involves driving to clients' homes / apartments; this is a community based role.
