Position Summary :
The System Navigation Specialist is responsible for providing direct support to households in the Priority Pool, ensuring they have the necessary documentation and guidance to maintain active enrollment and progress through the housing process. The role involves managing the client's master list, facilitating case conferencing, and delivering comprehensive case management and care coordination services. This position focuses on engagement, outreach, and support to reduce barriers to stable housing for individuals and families.
Essential Duties and Responsibilities :
Work with households in the Priority Pool to collect required documentation and ensure ongoing eligibility.
Provide clear guidance on maintaining active enrollment and next steps in the process.Offer system navigation services at the first point of contact to reduce the likelihood of losing contact with households.By-Name List ManagementMaintain and manage a By-Name list of households in collaboration with relevant providers.
Facilitate case conferencing with appropriate stakeholders to coordinate care and address housing barriers.Case Management and Care CoordinationUtilize motivational interviewing and active listening techniques to engage with households effectively.
Provide outreach and assistance with housing searches, including supportive or navigation services.Navigate the private rental market by building relationships with landlords and workforce development sectors.Refer households to workforce programs tailored to their needs, such as resume building, skill development, job seeking, and interview practice.Conduct document readiness activities for unsheltered households in compliance with established policies and procedures.Diversion ServicesDeliver services once households have been determined eligible for diversion.
Coordinate with service providers to ensure continuity of care and address any confusion regarding the process.Manage client load, provides timely and relevant services for clients, documents services in required systems in a timely and accurate manner, and provides outreach / educationClinically document client care coordination, in accordance to program / funder requirements and in HIPAA-compliant manner. Train new staff in the clinical care coordination documentation process.Position Requirements and Qualifications :
Education :
Bachelor's Degree in Social Work, Human Services, or a related field preferred; equivalent experience may be considered.Experience :
Minimum of 1 years of experience (Intern or Volunteer) in case management, housing services, or related field.Knowledge, Skills, and Abilities :
Familiarity with housing navigation, landlord engagement, and workforce development services.Strong interpersonal skills with the ability to engage clients through motivational interviewing and active listening.Knowledge of local housing resources, supportive services, and the private rental market.Ability to work collaboratively with diverse stakeholders, including landlords, service providers, and workforce programs.Proficient in data management systems and maintaining accurate records.Excellent organizational and time-management skills.Commitment to providing equitable and inclusive services to diverse populations.3-4 years as care coordinator, community health worker, navigator, counselor, or related role.Experience with under-served communities is a plus.Working ConditionsAbility to work in a fast-paced environment with competing priorities.Flexibility to travel locally to meet with clients, landlords, and service providers as needed.Occasional evening or weekend work may be required.Physical Requirements
