Client Information
Medical & Behavioral Background:
- Diagnosis: Mild Intellectual Disability, Bipolar Disorder
- Current Status: Hospitalized at Alta Bates, behaviorally unstable
- Medication: Recently adjusted; still stabilizing
Current Support Services
- Ligaya Day Program: Monday – Friday, 9 AM – 12 PM
- Redwood Developmental Services: Adaptive skills training, 12 PM – 4 PM
- Aviana Home Support: 1 PM – 8 PM
Care Needs & Preferences
- Schedule Required: Personal Assistance Sunday – Friday, 8 PM – 8 AM (12-hour shifts)
- Language Preference: Spanish-speaking caregiver
- Gender Preference: Female (due to nighttime assistance needs)
Daily Support Needed
- Dressing assistance
- Feeding support (to prevent choking)
- Bathroom assistance (wiping, pulling up pants)
