POSITION SUMMARY
This position is an integral part of the medical case management team that provides support for individuals who are living with HIV / AIDS. Works closely with the primary health care providers, state, and local agencies. This position will connect clients to necessary services including, but not limited to : Ryan White programs, housing and patient assistance, dental services, transportation, and more. This position delivers HIV prevention education to clients as well as administers HIV rapid testing and counseling services. This position is funded through Ryan White program grants as well as through the Medical Practice Association.
ESSENTIAL FUNCTIONS
Case Management
Enrolling Clients into Services
- Provides patients basic information about Ryan White programs (Parts B, C and D), ACA, patient assistance programs, housing, dental care, food banks, Medicaid and Medicare benefits, and other supportive services.
- Assists clients with enrolling into Affordable Care Act.
- Performs complete social service needs evaluations as indicated, including financial resources, support programs, transportation, nutritional assessments, etc.
- Maintains accurate and complete records of case management activities.
- Other duties as assigned.

POSITION QUALIFICATIONS
Certificates & Licenses :
Will be required to be certified as an HIV / AIDS Counselor and Educator (training provided).
Education :
Bachelor’s degree in social work, counseling, psychology, or other health related area. Relevant experience may be substituted for formal education.
Experience :
Microsoft Office experience a plus
