We are seeking an experienced and dedicated Home Health Intake Coordinator to join our team. Responsibilities:
- Handle inquiry calls with professionalism, providing detailed explanations of the assessment and intake process.
- Document all inquiry calls thoroughly, ensuring that all applicable patient information is entered into WellSky.
- Gather comprehensive information about the patient’s background, current situation, and health needs.
- Conduct patient interviews to assess their overall situation, identifying needs for treatment or support services.
- Make appropriate referrals and coordinate treatment or support services as needed.
- Prepare and process referral documents and intake reports accurately and in a timely manner.
- Ensure that all documentation is compliant with federal and state guidelines, maintaining accuracy and completeness.
- Maintain a patient-centered approach, always striving to do what is best for the patient and your coworkers.
- Adhere to company standards, policies, procedures, protocols, and all relevant laws and regulations.
- Demonstrate fiscal responsibility in managing supplies, ordering, and resource conservation.
- Complete all required training within designated timeframes as outlined by the company or leadership.
- Exhibit professionalism through reliable attendance and a positive, team-oriented attitude.
- Maintain strict confidentiality of patient and staff information in accordance with HIPAA and privacy laws.
- Effectively use Microsoft Office products (Excel, Word, Outlook) to complete daily reports and tasks.
- Manage commercial insurance verifications and processes.
- Contact patients to gather additional information and ensure smooth communication regarding their care.
