Nutritionist - RESPONSIBILITIES (listed are included but not limited to)
- The Nutritionist delivers medical nutrition therapy for both outpatients and, on occasion, inpatients, ensuring high-quality, patient-centered care.
- Lead diabetes education initiatives, including one-on-one counseling, group classes, and management of patients using advanced monitoring tools such as continuous glucose monitors and blood glucose monitors.
- Design and implement meal programs for adolescent wellness camps, incorporating nutrition education and promoting healthy lifestyle practices.
- The Nutritionist provides on-demand diabetes education for walk-in patients and collaborate with internal medicine and family medicine teams for seamless care.
- Maintain accurate and thorough clinical documentation in alignment with established standards and guidelines.
- Collaborate with an interdisciplinary diabetes education team, including certified educators, pharmacists, nurses, and technicians, to optimize patient outcomes.
- Partner with outpatient clinic teams to deliver continuity of care, addressing same-day patients.
- The Nutritionist performs both outpatient and inpatient public health nutrition services, including individual consultations and group education sessions.
Nutritionist - REQUIREMENTS
- Certification as a Certified Diabetes Care and Education Specialist (CDCES) is preferred.
- Minimum two years of experience in diabetes education for all ages, including gestational diabetes.
- Proof of flu and COVID-19 vaccinations.
- Current Registered Dietitian license in any state.
